Model Participation Agreement

Thank you for participating as a model for The Hair Care Edit – By Quiana Jones. A final copy will be available to be signed before the appointment.Model sessions are collaborative, creative experiences offered at a reduced or complimentary rate in exchange for portfolio development and promotional use. This agreement outlines expectations, responsibilities, and protections for both parties.By participating, you acknowledge and agree to the following:1. Purpose of Model Session
Model services are provided in exchange for:
• Creative flexibility
• Media documentation (photo and/or video)
• Promotional and educational usage rights
• Portfolio inclusionI understand that model sessions may involve technique demonstration, transformation services, or method showcasing. The stylist maintains professional and creative discretion throughout the appointment.2. Creative Direction & Outcome Expectations
I acknowledge that:
• Final results may vary from inspiration images.
• Technique selection and modifications are determined based on hair health, density, and suitability.
• Adjustments may be made during the appointment to protect the integrity of my natural hair.
• Model sessions prioritize technique execution and portfolio value, not full customization beyond agreed scope.While professional standards of care will be upheld, specific outcomes cannot be guaranteed.3. Hair Preparation Requirements
I agree to arrive:
• Fully detangled
• Free of prior extensions
• Without excessive oils, heavy product buildup, or residue
• With natural hair only (no leftover beads, tape, bonding, tracks, etc.)I understand that:
• A standard wash and blow dry is included.
• Excessive prep (extension removal, severe matting, residue breakdown, etc.) may result in additional charges.
• Failure to arrive prepared may result in cancellation or modification of the session.Proper preparation ensures we maximize transformation time and maintain schedule integrity.4. Media Release & Usage RightsI grant The Hair Care Edit – By Quiana Jones:
• Permission to photograph and/or record video during my appointment
• Permission to edit content for branding and educational purposes
• Permission to use media for website, social media, advertisements, digital marketing, print materials, and trainingI understand that:
• I will not receive additional compensation for media usage.
• Content may be used indefinitely unless otherwise agreed in writing.
• My participation implies consent to before-and-after documentation.5. Appointment Conduct & Professional EnvironmentI agree to:
• Arrive on time and remain for the full duration of the appointment
• Maintain respectful conduct
• Follow all in-salon guidelines
• Adhere to post-care instructions providedExcessive tardiness, or non-compliance may result in termination of the session.6. Extension & Transformation Risk DisclosureI understand that hair extension and transformation services may involve:
• Added tension to the natural hair
• Increased manipulation during installation and removal
• Temporary scalp sensitivity or discomfort
• Shedding or breakage if proper maintenance is not followed
• Potential stress to previously chemically treated or compromised hairI acknowledge that:
• Results depend on the integrity and health of my natural hair.
• I am responsible for following all aftercare and maintenance instructions.
• Failure to properly maintain extensions may result in tangling, matting, slippage, shedding, or breakage.
• Longevity and outcome vary based on lifestyle, maintenance habits, and environmental exposure.I confirm that:
• I have disclosed all prior chemical treatments (color, relaxer, keratin, etc.).
• I have disclosed any history of hair loss, thinning, traction alopecia, scalp sensitivity, or medical conditions affecting hair.
• Extensions are not recommended for severely compromised hair, and the stylist may decline service if deemed unsuitable.I voluntarily accept the risks associated with extension installation, wear, and removal.7. Liability Waiver
I acknowledge that all hair services carry inherent risks.I release and hold harmless The Hair Care Edit – By Quiana Jones from liability related to:• Allergic reactions
• Undisclosed prior chemical damage
• Pre-existing hair or scalp conditions
• Improper at-home maintenance
• Breakage resulting from failure to follow aftercareI confirm that all information provided regarding my hair history is accurate and complete.8. Cancellation & Participation Policy
Model appointments are reserved specifically for selected participants.
• A minimum of 48 hours notice is required to cancel or reschedule.
• Late cancellations or no-shows may result in disqualification from future model opportunities.
• Model appointments may not be transferred to another individual.
